Schedule II Disposal - DEA Form 222

The DEA Form 222 is the only legal way to transfer CII drugs from one location to another, either for purchase or for return. The DEA Form 222 is only used to request to receive drugs from a licensed distributor or pharmacy.

For example, to purchase CII drugs, a pharmacy must send a Form 222 to their wholesaler or a manufacturer, and to return drugs, the Return Service or manufacturer must send the pharmacy a Form 222.

To request to return CII drugs to Pharmaceutical Returns Service, one must fill out a Schedule II Return Request or otherwise provide the drug detail to our CII department. Pharmaceutical Returns will then send a Form 222 to request the drugs from the pharmacy. It is never legal to ship CII drugs without prior authorization in the form of a DEA Form 222.

Once you receive your DEA Form 222, federal regulations require that you follow these instructions carefully:

  • Do not alter any numbers printed or written on the form.
  • Check all information on the form for accuracy. If any portion is incorrect, please contact us at 1-800-215-5878 for instructions.
  • When all information on the form is correct, complete the packages shipped column and date shipped column for each item (if the entries were correct, the packages shipped should match the far left column "No. of Packages").
  • Photocopy the completed form and include the photocopy in the shipment. Ship only those pharmaceuticals listed on the Form 222 using the shipping label that arrives with the form, to PRS. Items do not need to be bagged as they are listed on the forms.
  • Send the green copy of the 222 to your local DEA office and keep the brown copy at your facility for at least two years following the date of issue.

PRS will issue a full report on your CII return within four weeks.
